Here's how to get your credit:
Visit AmazonLocal, and search for your city (if it isn't already in the box). If you already have an Amazon account, sign in. If not, you have to register.
Then, click on Get Voucher, and then complete the transaction. Voila! Instant credit!
There are some restrictions though: It is only for items $3.99 or more. It also expires on August 12, 2012. Limit one per household.
